Incident at 4723N 5117W


2015-09-23: UPDATE: TSB#A15A0063: The Cougar Helicopters Sikorsky S-92A (C-GFCH) was en route from St. John's, NL (CYYT) to the Grand Banks oil rig platform. After passing through heavy rain, the crew noted a dim illumination of the Master Fire Warning light. The number 2 engine Fire Light illuminated in the same manner. The lights became brighter and the aural ENG FIRE ALERT began. All indications were intermittent, however the crew decided to turn towards shore and call PAN PAN PAN for priority. Cougar Rescue 61 was launched, joined the returning flight and confirmed there was no visual indication of fire. All lights extinguished once the power was reduced to idle and the fire detection system functioned normally. The passengers were briefed continually on the status of the aircraft throughout the flight. Maintenance replaced the fire detector and cable assembly. Troubleshooting is ongoing.
2015-09-14: At 1227Z, approximately N4723/W5117, a Cougar Helicopters Sikorsky S-92A (C-GFCH/ CHI51) from St John', NL (CYYT) to an Oil Rig (GB53) transmitted PAN PAN PAN and declared an emergency. The flight advised they had a fire indicator in engine #2. The flight suspected an erroneous indicator. The flight requested to divert and was cleared direct CYYT. The flight landed without incident at 1259Z. Joint Rescue Coordination Centre (JRCC) Halifax advised.
2015-09-14: UPDATE: JRCC Halifax SARSUM Report#[S2015-00540]: (465400N 0514367W - 60 ESE CYYT). Gander Centre reported that a Sikorsky helicopter (Cougar 51) was 60 NM ESE of St. John's with 12 people on board with an indication of engine fire, and they were returning to St. John's airport. A 413 Squadron Hercules aircraft (R343) and a 103 Squadron Cormorant helicopter (R905) were tasked to assist. Both resources were stood down when the aircraft, under escort with another Cougar helicopter, landed safely at the airport. No further assistance required.
