Incident at ST. JOHN'S INTL NL (CYYT)
2015-06-23: Update from Maintenance & Manufacturing: Number 1 engine fuel pressure low caution illuminated several times during flight. On one occasion the engine 1 failure audio was heard accompanied by a brief flash of the engine out warning light. A roll on landing was carried out without incident. Health and Usage Management System (HUMS) determined that it was a momentary flameout and the engine automatically relit immediately. The following is a list of parts changed on the aircraft for snag isolation; the Fuel Filter was replaced and is in quarantine, the Breakaway Valve (slight wear was found), the Check Valve, the Fuel Manifold (slight wear found), and the Fuel Selector Valve. The aircraft has had a series of test flights and it is believed that the defect was caused by the fuel selector valve, based on the valve history in the S-76 fleet. The aircraft is being returned to service.
2015-06-03: TSB Report#A15A0028: The Cougar S92 (registration C-GMCH) helicopter was arriving at St John's International Airport (CYYT). While on short final to Runway 11, the number 1 engine (GE CT7-8A) fuel pressure low caution lamp illuminated several times. On one occasion, the engine 1 failure audio was heard with a brief flash of the engine out warning light. The engine did not fail completely and it completed an auto relight. A roll on landing was carried out without incident.
