Incident at HALIFAX / STANFIELD INTL NS (CYHZ) (Ground handling services)
2015-02-05: A Cargojet Airways Boeing 757-236 (C-FKCJ/ CJT619) from Halifax, NS (CYHZ) to Hamilton, ON (CYHM) requested emergency response services (ERS) at deicing facility due possible emergency power unit (EPU) fire. Crash alarm activated and ERS responded. At 0209Z, ERS advised situation under control, problem was traced to glycol being ingested into EPU. CJT619 departed at 0300Z.
2015-02-12: UPDATE: TSB Report#A15A0005: The Cargojet Boeing 757-200, C-FKCJ, was operating as CJT619 for a planned cargo flight from Halifax, NS (CYHZ) to Hamilton, ON. During de-icing operations at CYHZ, the crew received an aural fire warning sound with the associated forward panel and glare shield fire warning lights illuminated. Shortly after, fumes appeared on the cabin flight deck and in the main cabin. The crew contacted ATS and requested emergency rescue services (ERS) to assess. The crew noted that bleed air packs were inadvertently left open and immediately closed the packs, which immediately reduced the fumes inside the aircraft. The crew then shut down the APU as a precaution. ERS assessed and released the aircraft. Company maintenance inspection revealed that de-ice fluid ingested into the environmental system caused the fire warning and subsequent fumes.
