Incident at ST. JOHN'S INTL NL (CYYT)

Summary

2015-10-21: UPDATE: TSB Report#A13A0126: The Provincial Airlines Saab 340A aircraft (C-FPAI), operating as flight PVL900, departed St. John's airport on a local maintenance test flight. Take-off was conducted with the engine intake anti-ice system on. During the climb out the aircraft entered moderate precipitation and the crew noted sparks coming from the #2 engine anti-ice system. The crew carried out the quick reference handbook actions and selected the #2 engine anti-ice system off, no further sparks were observed. About 12 nm west of the airport the pilot requested priority to land as the aircraft was encountering moderate icing conditions. The aircraft landed without further incident. Company maintenance determined the intake de-ice boot was unserviceable and the sparks were likely due to moisture coming in contact with the exposed wires. The damaged portion of the de-ice boot was replaced and the aircraft returned to service.
2015-10-21: A PAL Airlines SAAB SF340A (C-FPAI/ PVL900) on a local flight from St. John's, NL (CYYT) departed runway 29 on a test flight and at 12nm west of the field, the pilot requested to return to the field. He also requested priority due to sparks from the anti-icing equipment on engine number 2. The problem was resolved after turning off the anti-icing equipment and the aircraft landed without incident at time 0158Z. No emergency declared and no assistance requested on the ground, however Jazz Aviation (JZA8630) was delayed approximately 2 minutes to accommodate the priority landing.
2015-10-21: UPDATE Maintenance & Manufacturing: The sparks were caused by an engine intake heat failure. The intake heat system experienced a short due to the exposure of the embedded wires. This is a known issue with this aircraft type. The operator has increased the inspection cycle and includes this system on all daily inspections resulting in a significantly decreased failure rate.
