Incident at WABUSH NL (CYWK) (ATS operating irregularity)


2010-09-30: UPDATE TSB (Quebec Region): A10Q0163 :The PSC437, Beech King Air 100 operated by Pascan Aviation, took off from Wabush bound for Baie Comeau. After takeoff, the aircraft was cleared to climb to 8000 feet on the 215 radial of the VOR YWK. At the same time, SPR912, a Saab 340 operated by Provincial Airlines, from Sept-Iles was descending to 9000 feet to Wabush on radial 194, VOR YWK. The controller cleared PSC437 keep the speed up to 17, without specifying 17 "DME". The pilot read back that he was cleared to climb to 17000 feet. The controller did not realize the error of interpretation in the readback. The error has been understood when PSC437 was reported at 10.5 DME climb to 17000 feet. The spacing should be 1000 feet vertical separation until the aircraft is 15 nm (17DME) and the two aircraft have at least 15 ° of divergence.
2010-09-24: A loss of separation occurred when PSC437, Beech 100, enroute from Wabush (CYWK) to Baie-Comeau (CYBC) was cleared through the altitude of SPR912, SF34, enroute from Sept-Îles (CYZV) to Wabush, without having received confirmation that it was an adequate distance from the CYWK VOR to ensure separation.
