Incident at HALIFAX / STANFIELD INTL NS (CYHZ)
2010-01-28: UPDATE TSB: A10A0001: CHI71, a Cougar Helicopters Sikorsky S-92, registration C-GVCH, was en route from Halifax to the Rowan Gorilla Three oil production platform when the crew declared a Pan, Pan, Pan due to illumination of the main gearbox (MGB) chip detector indicator. The aircraft returned to Halifax and landed safely without further incident. Maintenance inspection revealed a pin-shaped foreign metal object on the MGB chip detector plug. The MGB had just recently been overhauled. The operator will be replacing the MGB before returning the aircraft to service.
2010-01-04: CHI 71, Sikorsky S92A, enroute from the Raven Gorilla 3 platform to Halifax (CYHZ), 20 miles southeast of Halifax declared a PAN PAN PAN emergency due to a main gearbox chip indicator warning. Crash Fire Rescue and the duty manager were advised. The aircraft landed uneventfully on Runway 32 at 13:35Z. TSB Case Closed.
2010-01-04: UPDATE TSB: Significant amounts of metal fragments were found in the gearbox.. The gearbox will be replaced before the aircraft returns to service.
