Incident at CHARLOTTETOWN AIRPORT

Summary

2006-01-05: UPDATE Maintenance & Manufacturing: Maintenance QA reports there was no door warning light. The pilots noticed the fuel cap missing on the climb out from CYYG. The fuel cap was found on the threshold of Runway 03 which is basically direct path from fuel pumps to takeoff. There was no sign of damage to the fuel cap like it had been installed. Flight operations and SMS team are investigating this incident.
2005-12-19: CME301, Piper PA31, enroute from Charlottetown (CYYG) to Sydney (CYQY), departed Runway 03 at 1022Z. At 1024Z the pilot called saying he had a door indication and was returning to the airport. The pilot was given an airport advisory for Runway 03 and landed safely at 1028Z. The pilot called 15 minutes later and said that when doing his checks after landing, he noticed that the fuel cap was missing. A field check was conducted at 1046Z but nothing was found. Nil TSB.
