Incident at GREATER MONCTON ROMEO LEBLANC INTERNATIONAL AIRPORT (ATS operating irregularity)

Summary

2005-09-26: CME9732, Beech B99 enroute from Goose Bay (CYYR) to Moncton (CYQM) routing CYYR direct OLESU direct CYQM. Flight coordinated with CYUL at 8,000 ft. WICA. Aircraft was subsequently cleared to 12,000 ft. Re-clearance not indicated on flight data strip and no updated altitude forwarded to CYUL. No traffic. TSB Case Closed.
2005-09-28: TSB Update: A05A0129: The IFR flight from Goose Bay to Moncton was coordinated by Gander with Montreal at 8,000 feet. About ten minutes after departure, Goose Bay Ratcon coordinated 12,000 feet, Gander acknowledged the update but the flight's data strip was not been updated to 12,000 feet. The aircraft was subsequently cleared to 12,000; however, as the new altitude was not indicated on the flight data strip, the updated altitude was not forwarded to Montreal or Moncton. There was no loss of separation, and no conflicting traffic. Nav Canada is conducting an Operational Safety Investigation.
