Incident at 4749N 05354W

Summary

2005-02-22: TSB Update A05A0023: The AS-332L helicopter, C-GTCH, departed St. John's to carry out a long-line slinging operation in the Bull Arm area. On arrival at the site, an ""on site"" risk analysis was carried out between the customer and PIC. All flight and ground crew were briefed on: load description and weights, number of loads, signals, escape routes, for ground crew, approach and departure areas, and emergency procedures. Once all members of the sling team were briefed and precautionary measures were discussed and understood a trial run was conducted to the delivery area to assess site accessibility, weather conditions, visual acuity, and escape paths. The first load (2000 lbs) was delivered without incident or difficulty. The Second load (3500 lbs) was being delivered and while being set, came in contact with a sharp rock outcrop. The 180 foot ""Amsteel"" long line was cleanly severed 20 feet from the load. The load ended up short of the delivery area landing safely away from the upwind safety side of the delivery area. There was no recoil from long line when it severed and the operator reported that it was for this reason that they use the ""Amsteel"" long line. There was no damage to the helicopter or injuries to any of the personnel involved in the slinging operation. TSB Case Closed
