Incident at EDMONTON INTL AB (CYEG)

Summary

2003-09-04: UPDATE TSB reported that the United Parcel Service Boeing 747-100 was cruising at FL 370 about 40 NM west of Edmonton Intl (CYEG) when the number 4 engine, JT9D-7A/7Q, S/N 662317 flamed out. The crew descended to FL 280, then to FL 260 where they attempted unsuccessful restarts. After the engine was secured according the aircraft operating manual, The aircraft continued to its destination, Anchorage Intl (PANC). Maintenance was unable to determine the cause of the flame out, or duplicate the event and the aircraft was returned to service.
2003-09-02: UPS 6991, a B747-100 freighter, was 40 NM west of Edmonton on a flight from Louisville to Anchorage when the crew reported 1 engine shut down and requested descent to FL 280. The crew continued the flight to destination without further incident. TSB will contact the company.
