Incident at NORMAN WELLS NT (CYVQ) (ATS operating irregularity)


2003-08-19: A service vehicle had been cleared to work on and around Runway 09/27 by the FSS operator at 21:17Z. At 21:23Z the pilot of C-GXHO received an Airport Advisory for departure but was not told about the vehicle working on the field. The FSS Instructor (a trainee was with her at the time) scanned the field as C-GZHO was about to depart and did not observe the vehicle. The aircraft departed at 21:28Z without incident but the vehicle still had authorization to be on or near the runway and the driver questioned the FSS about the aircraft. In subsequent discussion with FSS, she admitted both her and the trainee had forgotten about the vehicle when they let the aircraft depart without advising it of it's presence.
2003-11-07: UPDATE ANS and Airspace reported that Nav Canada will hold an Operational Safety Investigation (OSI) concerning this occurrence.
