Incident at WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG)

Summary

2003-08-06: UPDATE TSB reported that C-FSLZ was departing Winnipeg for Norway House. Shortly after take-off as the aircraft was passing through 500 feet the crew noticed severe RPM fluctuations which were not controllable with use of the propeller speed lever. They declared an emergency and returned to Winnipeg for an uneventful landing. Company maintenance personnel were advised of the problem. (see A03C0196) In a second incident that day, TSB reported that C-FSLZ had just been returned to service from maintenance, concerning a previous occurrence (see A03C0195). Shortly after its first takeoff since maintenance release, the crew of C-FSLZ experienced uncontrollable RPM fluctuations. The crew diverted the flight to Winnipeg and landed without further incident. Company maintenance have not been able to duplicate the unstable RPM condition on the ground. The aircraft has been grounded pending more detailed propeller system troubleshooting.
2003-08-01: UPDATE Maintenance and Manufacturing advised that after T/O, climbing through 500' the right hand engine was observed at 103% RPM, retarding the power lever had no effect, once the prop sync was engaged the RPM fluctuated from 95 to 103%. The aircraft returned and landed without incident. There was a subsequent flight with the same problem and a number of test flights. After replacing the FCU, prop governor and numerous test flights the problem was resolved by correcting a binding eccentric shaft (when hot). The eccentric shaft is interconnection between the FCU, prop pitch control and prop governor.
2003-07-29: PAG 609, a Metro II, had just departed Runway 13 at Winnipeg when the crew advised ATC that they were returning to land due to a rough running right engine. Emergency Services was activated and the aircraft landed safely on Runway 31 and taxied to the apron. Another company Metro overshot to accommodate landing. No other aircraft were inconvenienced.
