Incident at WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG)
2003-07-15: CJT 571, a B727, was starting up on Apron 5 at Winnipeg when the crew advised Ground that they might have an engine fire. AFF was dispatched and while enroute, the crew reported that the fire was extinguished.
2003-08-07: UPDATE TSB reported that the Cargojet Airways Boeing 727-225, C-GCJB, was being started in preparation for a scheduled flight from Winnipeg to Vancouver, with a crew of three and one passenger on board. Engines No-1 and No-2 were started successfully. The start for No-3 engine was discontinued approximately twelve seconds after fuel flow commenced when there was no rise in exhaust gas temperature (EGT), and engines No-1 and No-2 (P&W JT8D) were shut down. Additional attempts to start engine No-3, and the fuel within the engine ignited, but the EGT rapidly approached the limits and the start was discontinued. During the start attempts, a small quantity of fuel had pooled within the engine and some had drained onto an air cart hose and the tarmac. When the engine start was discontinued, a tailpipe fire ensued, which also ignited a portion of the air cart hose. The crew contacted tower and requested that CFR respond. The ground crew pulled the air hose away from the aircraft and extinguished the the air hose and the tail pipe fire. The fire was extinguished before CFR could respond. There was no damage to the aircraft. Company maintenace personnel determined that the starting difficulty resulted from faulty igniters. The igniters for the No-3 engine were replaced and the aircraft was returned to service.
2003-07-16: UPDATE TSB reported that the a/c remained on the ground overnight and maintenance suspects that faulty engine igniters were the cause of the problem. Maintenance is waiting for delivery of replacement igniters to be installed.
