Incident at CALGARY INTL AB (CYYC)
2003-07-11: MPE 462, an F28, departed Calgary for Yellowknife with 36 people on board and when the flight was 8 NM northwest of the airport, the crew reported a fuel leak. ERS was requested and the flight returned and landed on Runway 16 without incident. The runway was quickly checked and re-opened after a few minutes. Departures were delayed about 5 minutes. TSB will contact the company.
2003-07-15: UPDATE Maintenance and Manufacturing reported that following refueling of the aircraft the fuel cap was not re-installed prior to flight. The fuel cap was left on the aircraft tire/wheel assembly. Upon push back, the fuel cap rolled off the tire and was run over by the same tire. Consequently upon removing the refuel hose from the single point connection, a small amount of fuel dripped out which is considered normal. Following takeoff, a ramp attendant found the crushed fuel cap beside the spilled fuel previously mentioned. Not knowing the severity of the situation, a call was made to Air NorTerra Operations Control, advising that fuel was leaking from the aircraft. The Captain was contacted and the crew burned off sufficient fuel to avoid an overweight landing. CFR was called out for landing. The landing was uneventful. Upon return to the ramp, Air Canada Jazz Maintenance found the fuel cap was indeed missing with no evidence of a fuel leak. The tire was also inspected with no fault found. The fuel cap was replaced and aircraft dispatched. An investigation has been done by the fuelling contractor and measures will be taken to ensure that fuel caps are not mislaid on the wheel assembly and are properly stowed in the refueling panel, then secured on the single point connection after refuelling is completed.
