Incident at EKATI NT (CYOA)

Summary

2003-06-05: UPDATE TSB advised that the flight crew noticed that the number 3 engine began to run rough, so they secured it as a precaution. Company maintenance discovered metal particles internally, so the engine was changed. The same a/c experienced some backfiring in that engine on June 3rd. Maintenance discovered that the valves were out of adjustment. The timing was adjusted and the a/c returned to service.
2003-06-03: C-GPSH, a Douglas C-54, departed Ekati for Yellowknife and when the flight was 50 NM south of Ekati, the crew did a precautionary shutdown of the number 3 engine. The flight continued to destination.
2003-06-13: UPDATE Maintenance and Manufacturing reported that on May 31, 2003 the # 3 engine was shut down in flight due to a power loss. The maintenance department determined the engine had metal contamination and the engine was removed and a replacement engine installed. The engine removed had a total time since overhaul of 181.0 hours and is being sent to an overhaul shop for investigation and a strip report. On June 3, 2003 the replacement # 3 engine was shut down in flight due to excessive backfire. Maintenance found a defective ignition lead and improper valve timing from the overhaul shop. The lead was replaced and the valves were timed, the aircraft has been returned to service. At the time of engine shut down the total time since overhaul was 4.8 hours. The overhaul shop was notified of the problem and an investigation is pending.
