Accident at MOLSON LAKE MB (CKJ8)

Summary

2003-06-02: C-FOKY, a Provincial Helicopters Bell 204 helicopter, was conducting a smoke patrol out of Norway House, MB with a pilot and 5 firefighters on board. Following vibration and banging sounds from the tail, the helicopter began to yaw and the pilot carried out a successful forced landing in a clearing. There were no injuries. Initial information is that a bolt on the tail rotor pitch link assembly failed. The company will remove the tail boom and install another one to fly the helicopter out of the bush. TSB is investigating and System Safety will assign a Minister's Observer.
2003-06-04: UPDATE TSB reported that the Provincial Helicopters Bell 204B, serial number 2043, registration C-FOKY had departed Norway House on a smoke patrol in support of forest fire fighting operations. The helicopter was established in cruise flight at 3,000 feet (2,200 feet agl) when, approximately 15 to 20 minutes into the flight, a banging noise was heard coming from the tail area followed by a slow and smooth 30 degree yaw to the left. The pilot gently applied opposite pedal and was able to correct the yaw. The banging was not repetitive, but appeared as pedal was applied. The pilot landed the helicopter straight ahead into a swampy area approximately 1/4 nm north of the Molsen Lake Lodge. The helicopter landed without incurring further damage or injury to the occupants. An examination of the helicopter revealed that one of the tail rotor pitch link bolts (AN 174-15 or subsequent) was missing and that the pitch link was hanging free from the tail rotor horn, but still attached on the opposite end to the crosshead assembly. One, or both of the tail rotors had struck the tail boom several times in flight causing the banging noise heard by the pilot and damage to the tail boom. The tail boom and tail rotor assembly is to be replaced in situ. More info to follow.
