Incident at FORT SMITH NT (CYSM)

Summary

2003-05-01: The crew of MEDEVAC AT 503#, a Beech King Air 200, was provided a clearance from Fort Smith (YSM) to City Centre Airport (YXD) with a restriction to cross 15 DME at 4,000 feet ASL or below. PLR 970, a Beech King Air 90, was inbound to YSM with descent clearance to 5,000 feet ASL. Neither the Yellowknife FSS or the crew of AT 503 could contact PLR 790 to determine the aircraft's position. FSS contacted the CARS O/C at Fort Smith, who indicated that PLR 790 was on final approach. AT 503's clearance was cancelled until PLR 790 landed at 2151Z. The pilot of PLR 790 indicated that he forgot to cancel the IFR. YSM METAR 2100Z 04005KT 15SM FEW220 07/M13 A 3005 RMK CS0
