Accident at FORT McMURRAY / MILDRED LAKE AB (CER4)

Summary

2003-03-12: UPDATE Maintenance and Manufacturing reported that the aircraft experienced a right propeller control or right propeller failure that resulted in the aircraft departing the runway. Both engines and propeller are damaged due to ground strike. The right propeller blade damage is consistent with a ground strike when the propeller blades were in a feathered position but facing 180 degrees opposite the normal feathered position. An SDR has been submitted with follow up reports as soon as the parts are examined internally to determine the cause of the failure.
2003-03-11: UPDATE TSB has upgraded the occurrence to an accident.
2003-03-10: UPDATE TSB reported that the Beech Super King Air 300 was on the landing roll at Fort McMurray/Mildred Lake, AB when the #1 propeller would not go into reverse. The aircraft skidded off the end of the gravel and snow covered runway at an approximate 30° angle, and struck a snowbank. There was damage to both propellers. There were no injuries to the 4 passengers or 2 crew members. Maintenance is enroute to the scene and will advise as to further actions. (Occurrence downgraded from accident to reportable incident.)
2003-03-07: C-FTLB, a Beech King King Air 300, landed on the 5,000-foot gravel airstrip at Mildred Lake and the crew lost control of the a/c after touchdown. The a/c exited the runway surface and entered a snowdrift. The a/c is currently stuck and the runway has been NOTAM'ed closed until further notice. Initial information from the TSB is that the number 1 propeller would not go into reverse after landing and as a result, the a/c veered off the slippery runway. Both props sustained damage after the a/c entered the snowdrift. The 2 crew and 4 passengers were not injured. TSB has given the owner permission to remove the a/c to the apron. The cause of the prop malfunction is not known.
