Incident at LA RONGE (BARBER FIELD) SK (CYVC) (Conflict - loss of separation, ATS operating irregularity)
2003-03-04: UPDATE ANS and Airspace reported that Nav Canada will conduct an Operational Safety Investigation (OSI) concerning this occurrence.
2003-02-17: TSB advised that a Cessna 441, C-GCTA, and a HS 748, C-FQVE, were both inbound for La Ronge from Prince Albert. C-FQVE was level at 12,000 feet ASL when the crew was issued an unrestricted approach clearance for La Ronge. The pilot of C-GCTA, at 16,000 ASL, requested descent and was cleared to maintain 12,000 feet ASL. C-FQVE had not reported leaving 12,000 feet and vertical separation was not assured.
2003-02-13: WEW 738 was at 12,000 feet and was given an unrestricted approach clearance and requestd to report leaving 5,000 feet. TW 441 was at 16,000 feet and requested descent and was issued a clearance to 12,000 feet. An ATC operating irregularity occurred when TW 441 was cleared to 12,000 feet and WEW 736 had not reported leaving that altitude. TW 441 subsequently canceled IFR through 12,000 feet. Vertical separation was not assured.
