Incident at MEDICINE HAT AB (CYXH) (Conflict - loss of separation, ATS operating irregularity)

Summary

2001-09-10: UPDATE ANS and Airspace reported that NAV CANADA will convene a Fact Finding Board.
2001-09-04: COA 1815, a B737, was enroute at FL 350 from Newark to Vancouver. ACA 150, an A320, was at FL 330, enroute from Vancouver to Montreal. The Calgary Enroute Controller cleared COA 1815 to FL 310. Approximately 20 miles southwest of Medicine Hat VOR, spacing between the 2 aircraft was reduced to approximately 2.7 miles with 1,300 feet vertical separation in an area where 5 miles lateral or 2,000 feet vertical is required.
