Incident at RED DEER REGIONAL AB (CYQF)

Summary

2001-08-10: UPDATE TSB reported that the pilot of C-FJZS, a Bellanca Decathalon, was 6 miles south of Red Deer at 3,000 feet above ground, practising aerobatics in preparation for an air show when the upper door separated from its hinges. The door entered the cabin forcefully and struck the pilot on the head, causing bleeding and disorientation. An emergency was declared and when the aircraft landed uneventfully at YQF, AFF was standing by to assist the pilot. The door hinge bolts will be examined in order to determine the cause of failure.
2001-08-07: At 2310Z, the pilot of C-GJZS, a Bellanca (Champion) 8KCAB Decathlon, declared a 'Mayday', stating that the aircraft door had come loose and struck him in the head causing bleeding and disorientation. The aircraft was in the Innisfail area where the pilot had been practicing aerobatices and the pilot was attempting to return to the Red Deer airport. C-GJZS landed safely on Runway 11 at 2319Z and was met by ERS. It was reported that the door was still on the pilot's head. He was later transported to hospital by ambulance. It was later reported that a bolt holding the door in place came out, resulting in the door being forced into the cockpit and striking the pilot.
