Incident at YELLOWKNIFE NT (CYZF)
2001-06-22: UPDATE TSB reported that the Consolidated Vultee PBY-5A was on approach to Defeat Lake, 20 NM east of Yellowknife when the right engine began to run rough. The flight crew rejected the approach and climbed to 1,000 feet. The right engine was shut down and the aircraft returned to the Yellowknife airport. The flight crew declared an emergency and requested AFF services for the arrival. The aircraft landed without further incident. Upon inspection, the number four cylinder on the Pratt and Whitney R-1830-92 engine had cracked from the spark plug hole to the intake valve. The right engine was replaced.
2001-06-20: The crew of TNKR 708, a Consolidated Vultee PBY-5A Catalina, declared an emergency at Yellowknife due to an engine shutdown at 2242Z, requested ERS, and landed safely on Runway 15 at 2252Z. TNKR 708 then required a tow clear of the runway. Six other aircraft were delayed 5 minutes each and one aircraft 10 minutes. TSB will be contacting the operator.
