Incident at BOYLE AB (CFM7)

Summary

2001-06-08: The pilot of the Bell 206B helicopter had just taken off when he noted the engine oil pressure fluctuating. The engine (an Allison 250-C20) then began to surge, at which time the pilot elected to return to the airport for a landing. While in a hover, just prior to landing, the engine failed. An autorotative landing was carried out. There was no damage to the aircraft and no injuries to the three people onboard. The reason for the engine failure is not known.
2001-06-11: UPDATE Maintenance and Manufacturing reported that during a routine flight for Forestry, the pilot felt a new vibration in the helicopter and observed the TOT gauge starting to rise. He decided to return to camp and had to reduce power to try and hold engine temperature down. Oil pressure started to fluctuate and while entering a hover, the engine quit about six feet above ground. No damage to aircraft except for the engine which appeared to suffer compressor damage, suspected FOD, which caused vibration and No. 1 bearing failure, which led to No. 1 seal failure, which led to loss of oil. Less than a quart of oil was left in the oil tank after landing. The engine has been removed and sent to an overhaul facility where it will be examined to determine exact cause of failure.
