Incident at STONY RAPIDS SK (CYSF)

Summary

2001-05-09: The pilot of C-GQXX, a Piper Navajo, initially reported an engine problem after departure from Stony Rapids and elected to return. After landing, the pilot advised that a fuel cap had come off during departure.
2001-05-11: UPDATE TSB reported that the Piper PA-31-350 was departing Stony Rapids when a passenger reported what appeared to be smoke from the right engine. Information provided indicated that the Captain also observed smoke and secured the engine. After landing at Stony Rapids it was determined, after a fuel cap was found missing, that the smoke was venting fuel.
