Incident at STONY RAPIDS SK (CYSF)
2001-04-30: The pilot of C-FSGD, a DHC-3 Turbo Otter, reported 9 NM west of Stoney Rapids (YSF) and received the airport advisory at 2250Z. The pilot then reported he had an engine failure and had landed safely on the ice 7.05 NM, bearing 265 degrees from Stoney Rapids. C-FVCK, C180 departed YSF at 0159Z to aid C-FSGD. C-FSGD was airborne off the ice at 0236Z. The pilot was on final for Runway 06 at 0242Z when another engine failure occurred. The pilot advised he could make the airport and landed safely at YSF at 0244Z. C-FGSD was towed off the runway at 0249Z. TSB has been in touch with the company.
2001-05-01: UPDATE TSB reported that the DHC-3 Turbo Otter was enroute from Fond Du Lac to Stony Rapids, when the fuel pressure dropped and the engine (P&W PT-6A-135) lost power. The pilot completed a forced landing on the river 5 NM west of Stony Rapids. The fuel guage indicated 50 gallons of fuel on board. The aircraft was examined and departed again for Stony Rapids. On final approach at Stony Rapids, the engine lost power again. The pilot completed a landing at the airport and the aircraft was towed off of the runway. No damage or injuries resulted from either landing. The aircraft was examined again and no usable amount of fuel was found on board. The operator has filed an SDR indicating a problem with the fuel transmitter.
