Incident at GILLAM MB (CYGX) (Conflict - loss of separation, ATS operating irregularity)


2001-01-11: UPDATE TSB reported that CAV 3743, was enroute from Winnipeg to Churchill under IFR. CAV 3743 had been flight planned at 15,000 feet, but to avoid headwinds, the crew obtained clearance to cruise initially at 7,000 feet and then at 5,000 feet while in controlled airspace. The controller calculated that CAV 3743 would pass overhead Gillam at 1030 CST, based on the forecast wind at the flight planned altitude of 15,000 feet. The estimate was not revised when CAV 3743 was cleared to cruise at the lower altitudes where the wind was substantially different. The crew of CAV 3743 reported to Winnipeg FSS abeam Island Lake at 0935, but did not provide an estimate for Gillam. The Cessna Citation, C-FEMA, was IFR from Winnipeg to Gillam at FL 330. The Controller did not have direct contact with CAV 3743 and was unable to establish radar or DME separation, so longitudinal separation was based on C-FEMA estimated to arrive Gillam at 1004, more than 15 minutes before CAV 3743 was estimated to enter controlled airspace 25 miles south of Gillam. C-FEMA was cleared for an approach to Gillam, with a restriction to maintain a northbound heading until 4,000 feet or below. C-FEMA landed at 1009. The separation standard in effect was 15 minutes longitudinal spacing. A loss of separation occurred when CAV 3743 reported passing overhead Gillam at 1011 at 5,000 feet.
2001-01-15: UPDATE ANS and Airspace reported that NAV CANADA will convene a Fact Finding Board.
2001-01-09: An Operating Irregularity occurred between CAV 3743, a HS 748, enroute from Winnipeg to Churchill via Gillam (YGX) V334 at 5,000 feet and C-FEMA, a Cessna Citation enroute from Winnipeg to Gillam direct YGX at FL 330 cleared to the YGX airport for an approach. CAV 3743 was estimated over YGX at 1630Z. C-FEMA was estimated at YGX at 1604Z. Longitudinal separation was based on C-FEMA estimated to arrive at YGX more that 15 minutes before CAV 3743 would enter controlled airspace 25 miles south of YGX. C-FEMA landed at YGX at 1609Z and CAV 3743 reported over YGX at 1611Z.
