Incident at GANDER INTL NL (CYQX) (Conflict - potential, ATS operating irregularity, ATM - NAVAIDS/radar)


2001-07-23: AMT8703, L101, on straight-in approach runway 31 when control tranferred to tower at seven (7) NM final. Immediately after control transfer Smoky50, (US AirForce), LJ35, cleared visual approach runway 31. Preliminary investigation indicates that planned separation may have been jeopardized. 2001-07-24: A01A0082:TSB UPDATE : An American Trans Air L1011 was on a straight-in approach for landing on runway 31 at Gander when control was transferred to the tower at seven miles. Gander radar was off the air for scheduled maintenance. A USAF LJ35 was also inbound to Gander from the north, and had been cleared to hold on the GOLF fix at 3200 feet because of the inbound L1011. Immediately after control of the L1011 was transferred to tower, the IFR controller then cleared the LJ35 for a visual approach, assuming that the tower would maintain separation. The GOLF fix positioned the LR35 for an extended base leg for runway 31 and as the aircraft was turning final, the crew, who had not yet switched to tower frequency, queried the center as to the position of the other aircraft; this was because they had been observing a TCAS target on the final approach course and that target was now behind them. The controller realized that there was conflict, and the LR35 was told to discontinue the approach and was turned to the left away from the runway centre line. During this exchange, the tower did not have visual contact with the LR35; the weather was VFR at the time. Minimum vertical separation was estimated to have been 1500 feet. NAVCANADA will be conducting and operational safety investigation. TSB Closed
