Incident at STEPHENVILLE NL (CYJT)

Summary

2001-07-10: KLM622 B767 enroute Atlanta to Amsterdam at 0012z, approx 80nm northwest of Stephenville KLM622 advised of PAN PAN PAN, advised declaring an emergency, and requested descent from FL330 to FL250 and direct to Gander for landing. Flight advised of pressurization problems and requested further descent to 10,000 feet. Once level at 10,000, KLM622 advised cancelling emergency. Everything under control, and no further assistance required. 188 SOB 23,000 KG FOB on landing KLM622 arrived Gander at 0050z. No ERS requested for arrival. TSB Case Closed.
2001-07-11: TSB Update July 10, 2001. A01A0073: THE CREW OF KLM 622, A BOEING 767-306 WITH 188 PEOPLE ON BOARD, DECLARED AN EMERGENCY WHILE APPROXIMATELY 80 NM NORTHWEST OF STEPHENVILLE, AND REQUESTED DESCENT FROM FL330 TO FL250 AND DIRECT TO CYQX FOR LANDING. THE CREW THEN ADVISED OF PRESSURISATION PROBLEMS AND REQUESTED A FURTHER DESCENT TO 10,000 FEET. THE CREW ADVISED CANCELLING THE EMERGENCY WHEN THE AIRCRAFT LEVELLED AT 10,000. THE FLIGHT LANDED UNEVENTFULLY AT CYQX. THE CREW LATER REPORTED A BLEED AIR DUCT LEAK INDICATION ON THE LEFT SIDE. THE PRESSURIZATION PROBLEM DID NOT REQUIRE THAT THE MASKS BE DROPPED. ROYAL DUTCH AIRLINES HAS BEEN ASKED TO PROVIDE THE TSB WITH FOLLOW-UP INFORMATION.
