Accident at ST. JOHN'S INTL NL (CYYT) (Aerodrome property - death/injury)


2000-06-11: CFGGJ PA34 declared an Emergency due to # 1 engine shutdown. CFS was called out. As CFGGJ was rolling out, the CFS vehicle "RED 3" was following along the runway. The driver of the vehicle lost control and the truck overturned on the runway. CFGGJ landed safely but runway 11/29 was closed til apprx. 2230Z due to truck. The driver was taken to hospital for observation and treated for minor injuries. TSB case closed. TSB file # AOOA0083. Maintenance personnel inspected the aircraft and found that a valve push rod in the engine had deformed. Update: The operator reported the oil leak and bent push rod at the #2 cylinder was the result of two nuts from the rocker arm wrist pin cover, at the cylinder head, had backed off.This allowed the wrist pin to move to the right, bending the push rod and affecting the valve opening thus creating the rough "running engine". Further inspection and repair are in progress and all engines with this style plate will be checked for retaining nuts security. An SDR has been prepared for submission.
