Incident at GANDER INTL NL (CYQX)

Summary

2000-04-19: CKS362 B747 departed Gander at 0333Z and in the climb reported he had shutdown #4 engine. When queried as to the nature of the problem the pilot replied that going through 14,000 ft. the #4 engine started gradually losing power until it shutdown. He said it would usually restart but this time he could not get it restarted. Acft levelled off at 22,000 ft.,continuing on to destination. Update TSB file # A00A0065. This is the second reported engine shutdown on this aircraft this year while operating in Canadian Airspace. The previous occurrence (12 March 2000, A00A0045) involved an uncontained failure on the # 2 engine. The operator has also had three other engine related reportable occurrences since 1998 while operating in Canadian Airspace (A98A0120, A99A0028, A99A0147) The NTSB has been advised of these occurrences for potential follow-up.
