Incident at Winnipeg, AB


1999-08-24: BLS 350, a Pilatus PC-12, was inbound IFR to Winnipeg from the Pas and had been cleared to desend to 5,000 feet. PAG 113, a Metro II, was cleared to take off from Runway 31 to maintain 4,000 feet IFR to Pine Dock. The clearance was read back correctly and at 1428Z the Arrival Controller advised the crew of BLS 350 that their traffic was a Metro II at the 1230 position at 6 miles northbound at 4,000 feet. Thirty seconds later, the departing aircraft was observed climbing above the assigned altitude. Both the Arrival and Departure Controllers took immediate action to vector each aircraft clear of the other. In the process, PAG 113 was cleared to continue the climb to the requested altitude of 15,000 feet. Radar data analysis revealed that PAG 113 was still 1,000 feet below BLS 350 when the lateral distance between them was 7.79 miles. At the time altitudes crossed at approximately 5,000 feet, lateral separation was approximately 5 miles. Lateral separation deteriorated to 3.07 miles with 300 feet vertical before lateral spacing began to increase. In a subsequent phone call from the pilot of PAG 113, he indicated that it was his fault the altitude restriction had been broken. During the cockpit conversation, he had called through 3,000 feet but missed the 3,800 foot call. OPI - System Safety UPDATE TSB reported that the crew of PAG 113 indicated they were having radio trouble at the time, got side-tracked into correcting the problem and missed the altitude alert.
