Incident at Yellowknife, NT

Summary

1999-08-23: At 2338Z, the crew of BFL 311, a Curtiss Commando, aborted take-off from Runway 27 at Yellowknife. CFR was called out in anticipation of the aircraft being unable to stop prior to the end of the runway. Although it appeared the aircraft had stopped short of the end of the runway, CFR reported the tall wheel left the runway surface by approximately 18 inches. No reported damage. The cause of the aborted take-off was a shifting of the load in the cargo bay. OPI - System Safety UPDATE Commercial and Business Aviation reported that two aborts were involved here. On the first take-off attempt, just as power was applied, the Captain looked back at the load and saw the pallet jack move. He aborted, advised Tower that they had to check something in the cargo area and taxied back for take-off. On the second take-off at 83 knots, a catastrophic cylinder failure occured which prompted the reject. The Captain did a 180 at the end of the runway at a speed of about 20 knots or so to preclude going off the end. In the process, the tailwheel swung off the runway. The Captain assures that the load did not shift. The FO admitted that he didn't properly secure the pallet jack prior to the first take-off attempt.
