Accident at Waterfound Lake, SK

Summary

1999-05-03: C-FASV, a DHC-3 Otter on skis, with a pilot and 5 passengers, was departing from the wet, slushy ice on Waterfound Lake to transport the passengers and their gear to a construction camp. The pilot elected to abort the takeoff when the a/c did not accelerate normally. He reported that the engine was producing 2100 RPM when it should have been 2250 RPM. After the pilot decided to reject the takeoff, the a/c ran up onto the shore into a stand of trees. Weather was reported to have been winds calm, temperature about +1 degree C. The occupants were able to evacuate the a/c without injuries, but the a/c caught fire and was destroyed. OPI - System Safety
