Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

Summary

1997-05-16: Aircraft departed runway 33 and blew a tire on the left side. The aircraft exited onto Golf taxiway. ERS was called out. Debris was cleared by 2230Z. The aircraft remained on the taxiway as there was material hanging from the gear. At 2310Z, they started to deplane the passengers since all the tires on the left side had to be changed. Ops. impact -- none. Supplemental information from T.S.B. Initial Notification (#A97O0065): During the take-off roll at approximately 115 knots, the crew received a "wrong configuration" take-off warning horn. The take-off was rejected without incident, however, during the taxi, the crew noted that the brake temperature exceeded 500 degrees Celcius on the port side. The crew discontinued taxiing and reported that the passengers would have to be deplaned where the aircraft had stopped on "G" taxiway. ATC services requested ERS equipment because of the hot brake situation. It was discovered that a speed brake handle micro-switch had caused the "wrong configuration" take-off warning horn. When the aircraft was inspected, it was discovered that two of the tires had deflated because of thermal discharge and a third had failed because the corresponding brake assembly had seized because of high temperatures. Maintenance replaced three tires on the port main wheel assembly, two brakes and performed an inspection of the undercarriage because of the high energy level required for the rejected take-off. The aircraft was returned to service.
