Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

Summary

1997-01-29: During the takeoff, a foward galley trolley came free from its stowage position and rolled down the right aisle. It struck a passenger in the front row of the ecnomy cabin. The passenger received a cut on the right leg. Investigation revealed the primary locking mechanism worked properly but it took effort to lock it completely. The secondary locking mechanism was found to function normally. Both the locking mechanisms are suppose to be in place when the trolley is in the stowed position. There was no damage to either locking mechanism indicative of any failure. Ops. Impact -- unknown. From a TSB Initial Notification.
