Incident at GRANDE PRAIRIE AB (CYQU)

Summary

1998-01-02: The pilot of C-FFBU, an AS 350 helicopter, had just lifted off at Grande Prairie and was transitioning to forward flight when a Shell fuel truck drove across the path of the helicopter. The driver of the truck braked and the pilot took evasive action to avoid a collision. It was reported that they missed each other by about 10 feet. There apparently is a blind spot in the area of the airport where this incident occurred, and both the pilot and driver did not see each other. The fuel truck was not in radio communication with the FSS, but the drivers normally monitor the radio frequency to listen for a/c movements. It is not known if there was a broadcast from the pilot before liftoff, or if the driver was listening to the radio. The incident happened after morning civil twilight, but before sunrise and the weather was good. This portion of the airport apron is also not visible from the FSS location. Both Shell and Canadian Helicopters have been contacted by System Safety to review radio procedures at the Grand Prairie airport. As a result, the helicopter operators, fuel suppliers, FSS staff and the airport manager will meet to review procedures to avoid similar incidents in the future. OPI - System Safety
