Summary

1996-12-19: BLS317 (Bearskin BE99), Sioux Lookout to Red Lake. PAG80 (BE95), Dryden to Red Lake. BLS317 was number one to approach, was issued the approach and turned over to FSS (Thunder Bay). PAG80, number two, was issued a holding clearance and turned over to FSS. At 1435Z, the FSS reported that BLS317 was down and PAG80 was on the approach. When question, the pilot of PAG80 believed he had received an approach clearance from FSS. FSS did not issue one. TSB were advised. Ops. impact -- unknown.
