Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (ATS operating irregularity)
Summary
1996-11-15: ACA485 was on approach to runway 06R. N930HA (Piedmont DH-8) was on runway 06R and was slower than expected to initiate their takeoff. ACA485 was instructed to overshoot and were subsequently given radar vectors to return on a right downwind leg for 06R. Whe N930HA took off, the crew was given a right turn to a heading of 100 degrees. As the two aircraft approached each other, both crews observed TCAS warnings and each crew had the other aircraft in sight. Both crews followed their respective TCAS instructions and the aircraft passed at the same altitude within 1/2 miles spacing in an area where 3 miles lateral or 1000' vertical is required. Ops.
