Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)
Summary
1996-08-27: ACA145 reportedly taxied to position on the button of runway 24R without clearance. The controller issued landing clearance to a cargo aircraft on final approach and then realized ACA145 was in position on the runway. The controller cleared ACA145 for immediate takeoff but the crew of the aircraft on final advised they were overshooting. The cargo aircraft overshot and ACA145 continued its takeoff without further incident. The weather was day VMC. Ops. Impact -- unknown. From a TSB Initial Notification. ATC reported that inconsistent radar returns from the ASDE and the fact that the runway threshold area is not visible from the tower cab contributed to the occurrence.
