Incident at TORONTO / BUTTONVILLE MUNICIPAL ON (CYKZ)

Summary

1999-06-08: The aircraft departed off runway 15 at 1033Z. After take-off and during the climb out, the pilot noticed an air draft coming from the left side of the aircraft. The pilot checked the vents and found that they were not open. As the aircraft levelled out at 1,500 feet, approximately two miles south of the airport on runway heading, the pilot checked the emergency exit window at his position. The window fell out and presumably fell to the ground. The pilot returned to the airport and landed without further incident. As of 1830Z, the aircraft window has not been recovered and there have not been any reports of damage from any ground stations. The area where the window fell is primarily commercial/industrial. Toronto Airways Limited Maintenance Department believes that the forward latch for the emergency exit may not have been properly closed, allowing wind pressure to force the window out. Ops. impact -- unknown.
