Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

Summary

1996-03-22: After landing in Toronto on a scheduled flight from London, England, the crew requested Air Canada maintenance to inspect the R-4 door because of a pressurization leak. There was a 30 cm structural crack in the door as well as several skin cracks. The aircraft was grounded. Air India engineering and Boeing were contacted to determine a suitable temporary repair. When the repair was completed, the aircraft was ferried back to India. The main structural crack appears to be an old crack which had been previously repaired in an unsatisfactory manner. The aircraft had been reportedly dispatched from London, England after the same pressurization problem had been inspected. From a TSB Initial Notification.
