Summary

1995-06-14: The 747 was preparing for departure, when cabin crew became aware of a customer agent still on board who needed to be deplaned. The bridge was extended without clearance from ground crew. The pushback was commenced and the aircraft door was pushed into the fuselage by the extended bridge. The aircraft's door and fuselage were damaged.
