Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)
Summary
1995-10-21: The aircraft was on an IFR flight from Vancouver International Airport (CYVR) to Toronto (LBPIA) (CYYZ). The flight crew reported that there was a severe nose wheel vibration after the rotation on take-off from Vancouver. The crew retracted the landing gear after the nose wheel vibration ceased. Inspection of the runway resulted in the discovery of a 12 inch piece of tread at the approximate position of the aircraft's rotation. The flight landed at Toronto (LBPIA) on runway 15 without further incident with ERS standing by. Maintenance inspection of the aircraft on arrival found the left nose wheel assembly had lost its tread. The aircraft taxied to the gate under its own power. Maintenance changed the left and right nose wheel assemblies during the overnight layover. Ops. impact -- unknown. 248 S.O.B. From a TSB Initial Notification.
