Incident at OTTAWA / MACDONALD-CARTIER INTL ON (CYOW)

Summary

1995-05-26: Aircraft was on its first flight since it was rebuilt. Shortly after lift off, at a height of 200' AGL, there was a loss of power. An autorotation was carried out, however, the tail rotor struck the ground, followed by flopping of the main rotor which severed the tail boom. The owner reported that the engine driveshaft coupling had failed, and that the pilot decayed the main rotor RPM before touchdown. Ops. Impact -- none.
