Summary
1995-05-23: The Tower Controller issued take-off clearance to the flight crew of C-GISH to depart Toronto (Buttonville) Municipal Airport (CYKZ) runway 21 without having received a release from the Toronto ACC Terminal Unit Controller. The Tower Controller was removed from duty, advised of the error and returned to duty. The Toronto (Buttonville) Tower Manager is continuing a preliminary investigation and securing related data. Ops. impact -- unknown.
