Incident at Moosonee, Ontario (ATS operating irregularity)
Summary
1997-04-16: AT 1300Z, C-FGWA contacted Timmins FSS to file an IFR flightplan. At that time, C-FGWA was on an IFR flight from YPO to YMO. No further contact was received. No reply was made to calls from Timmins FSS at 1326Z on 122.5. At 1340, CRQ818 reported final for runway 06 at Moosonee. C-FGWA then broadcast that he was on runway 06, backtracking, and would exit onto runway 32 to allow CRQ818 to land. At 1342Z, Toronto ACC called Timmins to advise that C-FGWA was on the ground in Moosonee. Preliminary investigation revealed that the FSS attempted to reach Toronto ACC unsuccessfully between 1326Z and 1328Z. When contact was established, the FSS did not request an update on the IFR estimate of 1334Z. The pilot advised that he was talking to Toronto ACC until 7 nm from Moosonee at which point he was cleared for the approach. His 2nd radio was being used to stay in contact with his company to get reports on the condition of two children who were being medevaced due to critical burns -- the mother was on board C-FGWA. As the controller did not specifically tell C-FGWA to contact Timmins FSS, the pilot did not. Toronto ACC is looking into the matter. C-FGWA did not conform to the IFR flight position reports order, therefore, an aircraft occurrence report is being filed. Ops. Impact -- CRQ818 was not aware of the position of C-FGWA.
