Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

Summary

1996-07-17: Prior to departing Ottawa, the flight crew completed an annunciator check and found no faults. Pushback and taxi were normal. Takeoff was normal, with no adverse handling of the aircraft noted by the crew. During the climb, the aircraft did not pressurize. The flight diverted to Montreal. During approach to Montreal, after flaps were selected down for landing, there was an aerodynamic "rumbling" felt/heard throughout the aircraft. All engine and aircraft parameters remained normal throughout the approach and the landing was completed without further incident. ERS were not called out. As the flight taxied to the gate, maintenance noticed that the aft cargo was in the full open and locked position and the door barrier was latched in the up position. Investigation revealed that both bulbs of the aft cargo door anunciator did not test. It is now thought that the panel malfunctioned during the pushback and the flight crew was unaware that the door warning light was not functioning. Ops. Impact -- unknown.
