Incident at Thunder Bay, Ontario (ATS operating irregularity)
Summary
1997-03-21: Bearskin 374 (Bearskin Lake Air Service) was inbound to Thunder Bay from Sioux Lookout and the crew had been cleared for visual approach to Rwy 12. Air Ontario 425 (Air Ontario Ltd) was cleared Thunder Bay to Winnipeg via J500 Sioux Narrows. Ontario 425 departed off Rwy 25 and was cleared direct Winnipeg. The crew turned right and in the process passed south of BLS374 with 2 miles lateral separation rather than the required 3 miles. Both aircraft were passing through 2600 ft at the time.
