1999-08-25: Aircraft accident approx 12NM NW of Sioux Lookout Airport between BL3362 Bearskin SW4 Fairchild enroute from Red Lake to Sioux Lookout IFR and SNY3103 Air Sandy PA31 Navajo enroute VFR from Sioux Lookout to Red Lake. Aircraft collided approx 12NM NW of Sioux Lookout. BL3362 - 3 SOB 2 crew plus 1 = 3 fatalities; SNY3103 - 5 SOB 1 crew plus 4 = 5 fatalities. Transport Canada Safety Board investigating. 2016-12-29: UPDATE: TSB Report#A95H0008: Air Sandy, a Piper PA-31 Navajo, with one crew and four passengers onboard had departed Sioux Lookout enroute to Red Lake. Bearskin Airways, a Fairchild Swearingen Metro 23, with two crew and one passenger, was inbound to Sioux Lookout on a flight from Red Lake. The two aircraft collided at 4,500 feet ASL, approximately 15 miles northwest of Sioux Lookout. The time of the occurrence was 1328 CDT. All eight occupants were fatally injured.
