1997-09-29: FMHD, Conquest C441, Sky Service NWA1100, Boeing B727, Northwest Airlines The controller on training FMHD (from Quebec to Dorval) behind ACA111 (from Halifax to Dorval) without restricting the speed. Spacing was at 2 miles instead of 4. NWA1100 (Detroit to Dorval) was turning too close to FMHD, causing a second loss of separation, 2 miles instead of 3.
